This presentation will discuss how IHSC’s clinical pharmacists have been pivotal in designing, developing and implementing clinical pharmacist roles within primary care that serves primarily the underserved. It will go through our policies, some of the tips and tools available to help advance the equity of care and then a candid portion that talks about how this plays out from a boots on the ground perspective. We will discuss trials, tribulations and successes.
